What Does Vitamin B12 Actually Do?
Vitamin B12 (cobalamin) is essential for three critical functions in your body: red blood cell formation, neurological function, and DNA synthesis. Without adequate B12, your body cannot produce healthy red blood cells, leading to a condition called megaloblastic anemia. Your nervous system depends on B12 for myelin production, the protective coating around your nerves. And every cell division in your body requires B12 for proper DNA replication.
The practical effects of adequate B12 are things you feel: sustained energy throughout the day, clear thinking and focus, stable mood, and efficient metabolism. When B12 is low, the opposite happens: persistent fatigue, brain fog, irritability, and sluggish metabolism.
Who is Most Likely to Be B12 Deficient?
B12 deficiency is more common than most people realize. The National Institutes of Health estimates that up to 15% of the general population may be B12 deficient, with rates significantly higher in certain groups.
Vegetarians and vegans are at the highest risk because B12 is found almost exclusively in animal products. Adults over 50 often have reduced stomach acid production, which impairs B12 absorption from food. People taking metformin (a common diabetes medication) or proton pump inhibitors (heartburn medications like omeprazole) are at elevated risk because both drugs interfere with B12 absorption. People with Crohn's disease, celiac disease, or other digestive conditions may also absorb B12 poorly.
Even if you eat a balanced diet, stress, alcohol consumption, and high-intensity exercise can increase your B12 requirements beyond what food alone provides.

You can buy B12 supplements at any pharmacy. So why get an injection? The answer is absorption.
Oral B12 supplements must survive your stomach acid, get absorbed through your intestinal wall, and make it into your bloodstream. At each step, you lose a percentage of the dose. Studies show that only about 1-2% of oral B12 is actually absorbed, and that rate drops further if you have any digestive issues.
A B12 injection bypasses digestion entirely. The full dose goes directly into muscle tissue and is absorbed into your bloodstream over the following hours. The result is faster onset, more reliable dosing, and noticeably stronger effects.
This does not mean oral supplements are useless. For maintenance, a daily sublingual (under-the-tongue) B12 can be helpful. But for correcting a deficiency or getting a noticeable boost in energy and focus, injections are more effective.
How Often Should You Get a B12 Shot?
For most people, a B12 injection every 1 to 2 weeks provides optimal results. B12 is a water-soluble vitamin, meaning your body uses what it needs and excretes the rest. There is no risk of overdose or accumulation.
If you are addressing a known deficiency, your provider may recommend weekly injections for the first month, then transition to every 2 weeks. For general wellness and energy maintenance, every 2 to 4 weeks is typical.
